Transitional Housing Initiative

The Transitional Housing Initiative is a program currently in development in Baltimore, Maryland, focused on providing safe, structured housing for adult men transitioning from incarceration or housing instability. This initiative is designed to support stability, accountability, and long-term independence during a critical rebuilding period.

What We Do

We offer short-term, supportive housing combined with wraparound guidance to help residents establish stability and work toward permanent housing. The program emphasizes structure, personal responsibility, and access to resources that support successful reentry.

• Case management and reentry navigation
• Life skills and personal development support
• Resource referrals for employment, healthcare, and community services

Individuals and referring partners can begin the process by completing the housing intake form. A team member will follow up to discuss eligibility, availability, and next steps.
